Yep, definitely a Scan Express 12000SP Plus - it says so all over the
documentation and the box it came in.

Nope. My scanner came with some really odd PCI SCSI adapter - a 1391
rather than the usual 1381 ISA card. I bought an INITIO model PCI 9100U
card. It works just fine.

The scanned image is shrunk in the x direction. That is to say, it
should be wider than it is.
