I'm using the 1220s with roughly the same card. There's a brief blurb

At the end of my /etc/rc.d/rc.local I added the following line:
/sbin/insmod g_NCR5380 ncr_irq=254 ncr_addr=0x220 dtc_3181e=1
Your mileage may vary - I used ncr_irq=254 because I wanted the irq to
be autoprobed though I don't think it even has an irq, and the ncr_addr
for my particular card was 0x220 - if 0x280 gets funky with you try the
others listed: "can be: 0x220 0x240 0x280 0x2A0 0x2C0 0x300 0x320 0x340"
Do the insmod manually from outside of X (so if it locks up you can at
least go to another terminal and rmmod or kill the other session). I
locked up X while trying to get the 0x280 to work. Your card may be
different, so try a few until you get the right one. These cards are
notoriously simple which means a simple autoprobe doesn't necessarily
turn up anything useful. But I refused to purchase another card (well,
my wife refused to let me) so with a little tinkering, I got mine to
work.
