I have seen such behavior on even SCSI-II systems if passive terminators
were used (not with a scanner). Switched to active terminators and the
flakiness went away. I read somewhere that the SCSI-II (and I presume
SCSI-III) standard requires active termination. I figure it's worth the
$3-4 difference to eliminate at least that as a possible source of
problems.
